Create form templates, collect submissions, and set up approval workflows.
What Are Forms?
Forms in Thicket let you create structured templates for collecting information. They're ideal for employee onboarding, change requests, incident reports, and approval workflows.
Creating a Form Template
- Go to Forms in the sidebar
- Click New Form
- Set up your form:
- Name — the form title
- Description — what this form is for
- Fields — add text inputs, dropdowns, checkboxes, date pickers, and more
- Configure approval workflows if needed
- Publish the form
Field Types
Forms support various field types:
- Text — short and long text inputs
- Number — numeric values
- Date — date picker
- Select — dropdown with predefined options
- Checkbox — yes/no toggles
- File upload — attach documents
Submissions
When someone fills out a form:
- They click on the form and fill in the fields
- Submit the form
- The submission appears in the form's submission list
- Approvers are notified if an approval workflow is configured
Approval Workflows
- Set up single or multi-step approval chains
- Approvers receive notifications when a submission needs review
- Each step can approve, reject, or request changes
- Track the status of every submission
Viewing Submissions
- View all submissions for a form in a list view
- Filter by status (pending, approved, rejected)
- Export submission data for reporting
